ABOUT US
We are a small general dentistry practice located on the north side of Chicago. We offer many services from the regular cleaning and exam to more specialized cases. We're a family practice that is happy to work with your whole family from back to school exams all the way to dentures.

Dr. Demetrakakis opened his practice in 1992 after graduating from Loyola University Chicago, and then continued on to dental school at The University of Illinois at Chicago. He lives nearby and believes in helping his community. Patients that start with Dr. Demetrakakis tend to stick with him for many years even after moving out of the neighborhood.
We are in network with Delta Dental, Sun Life, Mutual of Omaha, and United Concordia insurance plans. We also work with many PPO plans through other carriers. We are not affiliated with any HMO, Medicare, or Medicaid plans. **We offer free 2 hour parking! Park in the garage at The Breakers (5333 N. Sheridan). Just bring the ticket to your appointment and we will validate it.**

To make it a little easier for you, please print and complete the first form shown here titled, "New Patient" before coming to your first appointment. Each patient needs their own form. If the appointment is for a child under the age of 16, please print and complete the "New Pediatric Patient".
